If you opt for one of the Standard and Hardcore Ladders you'll be playing only the four original acts in the game Diablo 4 Gold. This means that the rankings only take into consideration the time you begin the game until you take down Diablo in Act IV.
In Standard Expansion and Harcore Expansion Ladders, players would have to run through five acts taking into account an expansion called the Lord of Destruction expansion.
